Amber is Excellent at Connecting Care for Complex Patients

Amber Zolp has been in the Ambulatory Care Manager role for just over a year. In this short time, she has excelled so much, enrolling dozens of patients in our chronic care management program. This allows the primary care providers to better follow these patients who have complex medical needs and provide them with the best care possible. Amber is always conferencing with other members of the care team including social work and the pharmacy to determine the best plan and resources for her patients.
