Amber is Excellent at Connecting Care for Complex Patients

Amber Zolp has been in the Ambulatory Care Manager role for just over a year. In this short time, she has excelled so much, enrolling dozens of patients in our chronic care management program. This allows the primary care providers to better follow these patients who have complex medical needs and provide them with the best care possible. Amber is always conferencing with other members of the care team including social work and the pharmacy to determine the best plan and resources for her patients. She is truly dedicated to her patients and practice. Please see some statements from Amber's providers and coworkers:

Amber is proactive in enhancing patient care, recognizing areas that need attention, and working toward effective solutions. Despite managing her own caseload, she is always willing to help when needed, such as assisting with a wound patient when the wound vac malfunctioned during an appointment. She offers a second set of eyes, shares her expertise and provides support without hesitation. Even when busy, she prioritizes patient care, ensuring they receive the best possible support and resources. I don't know what we would do without her."
