Advance Care Planning (ACP) is a process of personal reflection where you consider the types of life-sustaining treatments that would be acceptable for you. You select (name) patient advocate(s) and share the types of care you would want in the future if you are ever unable to speak for yourself. Part of the process is to complete an advance directive form and have honest discussions with your loved ones.
