Surgical Options for Loose Skin Removal after Weight Loss

After significant weight loss, it is common to be left with loose skin. In many cases, this loose skin can cause issues like skin rashes, abrasions or wounds. While symptoms can often be managed with drying powders, medicated ointments and support garments, these treatments don’t always work. In cases like this, plastic surgery is often the best option to provide lasting relief. In addition to being medically necessary, many skin removal procedures also have cosmetic benefits.

Plastic surgery procedures that can remove excess, loose and saggy skin include:

Arm pits and underarms

  • Brachioplasty: Also known as an arm lift or “bat wing surgery”, this procedure removes excess skin and fat from the upper arm and in the armpit region.

    The number one goal of a brachioplasty is to tighten loose, saggy skin. It can also help tighten supportive tissue and smooth over the remaining skin. It is common to get liposuction at the same time to reduce fat pockets and achieve a more sculpted, toned look.

It is important to know that this procedure will leave a large scar. Therefore, you should consider your reasons for having this surgery and have an open discussion with a plastic surgeon about your expectations.

Stomach and waist

  • Abdominal panniculectomy: This procedure involves removing the "apron" of excess skin and fatty tissue, known as the pannus, which typically develops along the lower abdomen/beltline after significant weight loss.
  • Abdominoplasty: Also known as a tummy tuck, this procedure removes excess skin and fat from the abdomen in the belly button area. It also focuses on tightening the abdominal muscles, for a more toned appearance. This is commonly performed in addition to an abdominal panniculectomy.

The incision scars from both procedures are permanent, though they are typically along the bikini line, keeping them hidden.

Patients can also have loose skin removed from the upper abdomen under the breasts.

Inner thighs

  • Thighplasty: Also known as a thigh lift, there are different forms of the procedure that can be performed based on an individual’s situation. An inner (medial) thigh lift removes excess fat and loose skin between the upper legs.

The scar from this procedure starts in the groin area and extends down toward the knee or the back of the thigh.

When should someone consider loose skin removal surgeries?

It is possible for skin to stretch after excess skin removal surgeries. Because of this, it is recommended they be done only after one has hit their lowest weight and has maintained that weight for six months. Results are better and complications are fewer when the skin envelope is fully deflated in appearance.

It is also important to keep in mind that, though these procedures are common and most patients are highly satisfied with the results, these are major surgeries. Complications are always possible and scarring is unavoidable and permanent. A plastic surgeon will discuss goals, expectations and the best option for each individual.

What can one expect from surgery and recovery?

Most of the time, loose skin removal is done on an outpatient surgery basis. This means that it does not require an overnight stay in the hospital. However, full recovery at home may take several weeks.

Does insurance cover loose skin removal?

When skin removal is done for medical purposes, insurance may cover some or all of the costs. If only done for cosmetic purposes, most insurance companies will not cover these procedures. Your first step is to talk with your insurance company about your policy and coverage. If covered, photos and supporting records from a primary care doctor will most likely be required.
