The Bronson Advanced Medicine Clinic provides specialized pre-surgical and follow-up care for patients with complex medical needs.
Patients are typically referred to the clinic either before surgery to help optimize their health and reduce risk factors such as high blood pressure or after a hospital stay to support recovery and prevent readmission.
During each visit, patients meet with a hospitalist who reviews their medical history, discusses treatment or procedure plans and ensures all questions are answered. The team focuses on helping patients manage chronic conditions and stabilize their health following hospitalization.
Common conditions seen in the clinic include:
- Congestive heart failure (CHF)
- Chronic obstructive pulmonary disease (COPD)
- Pneumonia
- Sepsis
- Newly diagnosed diabetes
The clinic also serves patients who do not have a primary care provider (PCP) or those unable to get a timely follow-up appointment after discharge. All recently discharged patients are encouraged to see a provider within seven days of leaving the hospital to ensure continuity of care and reduce the likelihood of readmission.
