Call our office for an appointment. Please come to the office 15 minutes before your appointment. Be sure to tell our staff if your lab test results and X-rays need to be sent to other doctors or medical practices. Follow-up appointments are made before you leave the office.
If you are unable to keep your appointment, 48 hours notice is required. Call (269) 349-2266 and press 4 to cancel and/or reschedule your appointment. If you fail to notify us, there may be a no show/cancellation fee. See Fees & Insurance for details.
If you are late for your appointment, we may ask you to change to another time.
There are times when we may need to change your appointment due to an emergency. We are sorry for any problems this may cause.
New patients are generally seen within 4-6 weeks. However, if your problem is of an urgent nature, you will be scheduled as soon as possible.
Your First Appointment
A referral from your family doctor may be needed for your first visit.
Our doctor needs to know about your health history to provide the best care for you. You should receive a new patient packet at least 10 days prior to your visit. Please complete and mail the forms back to us in the envelope provided. If you do not get them before your visit, come to the office 15 minutes early to complete the forms.
Your first visit will include a review of your health history, medical records and a physical exam. Bring a list of any medicines that you are taking. Make sure we have any of your other medical records.
A letter will be sent to your family doctor regarding the results of your visit. Your family doctor will still take care of you for any of your other conditions.
Canceling an Appointment
We know things may happen that make it hard to keep your appointment. If you are unable to keep your appointment, 48 hours notice is required. Call (269) 349-2266 and press 4 to cancel and/or reschedule your appointment. This allows other patients to see the provider.
