For your convenience, our office is open Monday through Friday, from 7 a.m. to 6 p.m. Follow-up appointments are made before you leave the office. For added convenience, we provide lab services and radiology in our office.
If you have an emergency, allergic reactions, severe burns, broken bones, head injury, loss of consciousness, poisoning and overdose, severe bleeding or chest pains, please go to an urgent care facility or the emergency room. For problems such as colds, flu, headache, back pain, slight fever, sore throat, diarrhea, please call our office. Same day appointments are offered with support from our mid-level providers. If you are late for your appointment, we may ask you to change to another time.
Your First Appointment
Your doctor needs to know about your health history to provide the best care for you. Before your first appointment, you will be mailed forms to help us assess your health history. Please arrive 15 minutes early to register with the completed paperwork. Please bring all of your medication bottles with you to each appointment and an updated summary from other providers you see, including seasonal providers.
Canceling an Appointment
We understand it may be necessary for you to cancel an appointment. Please notify our office 24 hours in advance, which will allow us to utilize the appointment time for another individual who needs to be seen. If you do not call 24 hours in advance or you are late for a visit, there may be a charge. For multiple incidents you may be requested to find another provider.
