Medical Records

Your medical record is confidential. We will not talk about your condition with anyone without your permission. If you wish to release information from your medical records, complete a Release of Information form and return to our office.

Hospitalization

Our doctors, nurse practitioners and physician assistants only see patients at our office. If you need to go to the hospital, you will be cared for by a doctor at that hospital. If you are admitted to a Bronson, another doctor will take care of you and keep your Bronson LakeView Family Care provider aware of treatment of your illness or injury. After you leave the hospital, you will see your Bronson LakeView Family Care provider for follow-up care.

Before your first visit, you need to fill out your new patient health history forms in Bronson MyChart. You and your provider will talk about your health history and medicine during your appointment. This helps you get the best possible care. Sign up for Bronson MyChart to get started.
