Your First Appointment
You must have an appointment and a referral from your primary care physician to visit our office. Please try to call as far in advance as possible. To provide the best care, your provider must know as much as possible about your health history. We will ask you to fill out patient registration and health history forms before your first visit. These forms may be mailed to you before your visit. Or you may plan to arrive 15 minutes early to fill out the paperwork.
Your first visit will include a review of your health history and a physical exam. It may also include a review of laboratory and X-ray studies.
Bring a list of any medicine that you may be taking, including over-the-counter medications and supplements.
Our specialists will send a report of your visit to your primary care provider. Communication with your healthcare provider regarding a plan of care is important.
