Infusion

Referral Guidelines

To begin the referral process and scheduling of the patient, the referral coordinator team requires a completed medication form, clinical notes, lab and or testing results, and patient demographics. The referral coordinator team will work to obtain an authorization for treatment. The patient will be called to schedule their appointment once this has been completed.

Fax the following:

Completed medication order signed by prescribing MD or DO including NPI number (note: NP and PA may only sign orders if they have privileges at Bronson Healthcare).
Recent clinical notes
Lab results or test results if required for medication administration or insurance approval
Patient demographics including a copy of their insurance card

Primary Care Scheduling

Most primary care offices do not require a referral. Visit our Find a Doctor to view every primary care provider who is open to new patients to schedule online. You can also schedule by phone and talk with a Bronson Care Advisor, Monday through Friday, 8 a.m. to 6 p.m. Or email us 24/7 at careadvisor@bronsonhg.org. If you call after hours, you can leave a message and we will call you back the next business day.
