Bronson Medical Group Primary Care Practices Recognized for Providing Exceptional Care

With more than 700 physicians and advanced practice providers, Bronson Medical Group is the largest multispecialty medical group in southwest Michigan. We are pleased to share two recent accomplishments that showcase the high-quality care our primary care practices provide to community members in Calhoun, Kalamazoo and Van Buren counties.

BLUE CROSS BLUE SHIELD PATIENT-CENTERED MEDICAL HOME DESIGNATION

There are roughly 1,600 primary care practices in Michigan that are designated as a Patient-Centered Medical Home (PCMH) by Blue Cross Blue Shield of Michigan. This year, 30 Bronson primary care practices met the high standards for PCMH designation. Four Bronson practices ranked in the top 10; Bronson Internal Medicine – Portage was ranked number one.

A PCMH is a care team, led by a physician, that looks at a patient’s whole health picture, focuses on each patient’s individual health needs and coordinates their care across all settings. It’s a model of care that puts patients at the forefront, building better relationships between patients and their care teams.

Bronson practices ranked in the top 10 in the state:

• Bronson Internal Medicine – Stadium Dr.
• Bronson Internal Medicine – John St.
• Bronson Rambling Road Pediatrics

Research shows that PCMH-designated practices deliver higher-quality, more preventive care than non-PCMH practices while also helping to reduce healthcare costs, hospital re-admissions and emergency room visits. In 2024, PCMH-designated practices had a:

• 85% higher rate of childhood immunizations
• 45% higher rate of pediatric weight assessment counseling with BMI
• 44% higher rate of colorectal screening
• 31% higher rate of breast cancer screening
• 61% higher rate of controlling blood pressure

PRIORITY HEALTH 2023 QUALITY AWARDS

Each year, Priority Health recognizes top primary care offices across Michigan that emphasize their commitment to high-quality, cost-effective care, prioritizing patient outcomes.  

These awards recognize practices that receive the highest scoring on the Priority Health Primary Care Provider Incentive Program. Practices must meet or exceed the national 90^th percentile across 13 performance measures in preventive care, patient satisfaction and chronic disease measures, including cancer screening, immunizations and chronic condition management. Awards presented in 2024 honor provider performance in 2023.
