REPORT OF RECORDS BREACH
On August 9, 2012, the personal laptop of Ennis Berker, PhD, a psychologist in private practice, was stolen. Bronson has learned this provider’s computer was not password protected or encrypted and that it contained information about patients seen by Dr. Berker on the campus of Bronson Methodist Hospital from 2004-2008, and since 2009 at Bronson LakeView Hospital (Paw Paw) and Bronson Nursing and Rehabilitation Center (Lawton). To date, there is no indication that the information has been accessed or used by an unauthorized individual.
Bronson is committed to providing quality care, including protecting patients’ personal information. We are issuing this notice to assure the public that we have policies and procedures in place that we regularly monitor to protect patient privacy including encryption of all hospital-owned electronic devices. Bronson’s electronic health record, Epic, is both password protected and encrypted. We are taking steps to further educate independent providers about how to protect data on their personal and office computers to prevent breaches of patient information.
Bronson has sent letters to 698 individuals informing them that personal information they provided to Dr. Berker may have been compromised. That information includes their name, date of birth, and reports describing the findings of psychological evaluations including diagnosis and recommendations for treatment. The stolen laptop did not contain any hospital medical records or access to Bronson’s electronic health record. Nor did it contain addresses, social security numbers or any financial information.
While the risk of identity theft from this incident is low, we have informed those affected that they may choose to place an alert on their credit report by contacting one of these companies: Equifax Credit Information Services at (800) 525-6285; Experian at (888) 397-3742 or TransUnion at (800) 680-7289.
