Your First Appointment

The doctor needs to know about your health history to provide the best care for you. These health history forms will be mailed to you along with any special instructions to prepare for the visit. If you did not complete the forms, please come to the office 15 minutes early to complete the forms.

Your first visit will include a review of your family health history, medical records and a physical exam. Bring a list of any medicines that you are taking. Make sure we have any of your other medical records. A letter will be sent to your family doctor regarding the results of your visit. Your family doctor will still take care of you for any of your other conditions.

You should let your insurance company know why you need to see the doctor. This will help you get the best benefit from your insurance plan. It is important for you to get authorization before your visit. If youdo not get your approval before your appointment, you may have to pay for the visit yourself.
