Behavioral Health Collaborative Care (BHCC)
Your mental health is an important piece of your overall wellbeing. Unfortunately, finding mental health services can be challenging. That's why we adopted this model of care - to help patients find quicker access to mental health conditions, such as depression and anxiety.
What is Behavioral Health Collaborative Care (BHCC)?
An evidence-based, patient-centered program offered at Bronson OB practices and select primary care practices, for patients 12 and older. In partnership with a psychiatrist, the program helps primary care providers treat patients with common mental health conditions, including:
- Anxiety
- Depression
- Suicidal Thoughts*
* further assessment to determine best treatment option
This model of care is twice as effective and most likely costs less, compared to traditional care.
How does the program work?
If you are suffering from anxiety, depression or have other mental health concerns, your provider may recommend you meet with a behavioral health specialist.
- The behavioral health specialist will evaluate your symptoms and then review your concerns with a licensed psychiatrist at Western Michigan School of Medicine (WMed) or Grove Emotional Health Collaborative.
- The psychiatrist, behavioral health specialist and your care provider will recommend a treatment plan.
- Your care provider will manage and prescribe agreed upon medications.
- The behavioral health specialist will check in with you on a regular basis to monitor your symptoms and progress and provide additional support if needed.
Is this program covered by insurance?
BHCC is a billable service provided by Bronson. Our behavioral health specialist will provide additional information about billing to further discuss with your health insurance provider to determine coverage prior to scheduling. The cost of our services will likely be less than if you were to seek behavioral health services outside of our program.

Bethany Adams, LMSW

Bethany brings a wealth of knowledge and over 15 years of experience in mental health, specializing in the care for adolescents and young adults. She has been serving patients and families at Bronson for more than a decade.
She is passionate about helping teens navigate the unique challenges of adolescence. Using evidence-based practices, she creates a safe and supportive space for her patients to explore concerns, build coping skills and strengthen resilience. Her goal is to empower young individuals to lead healthy, fulfilling lives. Bethany earned her master’s degree from Western Michigan University.
Kimberly Baker, LMSW

Kimberly is a Licensed Master Social Worker with over 20 years of experience supporting individuals through anxiety, depression, grief, and overall life stress. For the past eight years, Kimberly has been with Bronson, the last four in the Behavioral Health Collaborative Care program.
She brings advanced training in cognitive behavioral therapy (CBT), dialectical behavioral therapy (DBT) skills, motivational interviewing (MI), behavioral activation, problem-solving, and solution-focused brief therapy—allowing her to personalize care to each patient’s needs.
Kimberly’s approach is rooted in the belief that healing is possible. She helps patients build confidence, develop practical tools and take meaningful steps toward emotional wellness. She earned her bachelor’s degree in Clinical Psychology from Michigan State University and a master’s degree in Social Work from the University of Michigan.
Hailey Conklin, LMSW

Hailey has five years of experience in medical social work, primarily in the case management setting. She is passionate about helping patients set meaningful goals and discover strategies that support both mind and body.
Hailey believes in a collaborative approach to care, guiding patients toward feeling their best and living a life of wellness. She earned her Master’s degree in Social Work from Western Michigan University and has supported individuals from diverse backgrounds through case management and care coordination.
Jennifer Fisher, LMSW

Jennifer is a Licensed Master Social Worker and Certified Advanced Addiction Drug Counselor with over 15 years of experience supporting adolescents facing anxiety, depression, trauma, and school-related challenges.
At Bronson, Jennifer brings a deep understanding of how physical and mental health intersect in young people. She enjoys helping teens explore how their bodies and minds work together and guiding them through small, meaningful changes to improve mood and manage anxiety.
Jennifer is committed to partnering with each patient to set personalized goals and build a path toward mental wellness. She earned her master’s degree in Social Work from Western Michigan University and has dedicated her career to working with youth and their families.
Sarah Frederick, LMSW

Sarah is an experienced social worker with a passion for supporting adult and geriatric patients. She spent six years working in skilled nursing facilities prior to coming to Bronson, where she’s provided care in both hospital and primary care settings, for over 22 years.
For the past four years, Sarah been part of the Behavioral Health Collaborative Care team. She brings a wide range of experience and is committed to meeting patients where they are—helping them set meaningful goals and tailoring care to their individual needs.
Sarah values the flexibility of connecting with patients through video, phone, MyChart, and in-person visits. She earned both her undergraduate and master’s degrees in Social Work from Western Michigan University, with additional studies in Sociology and Gerontology.
Cynthia Jubenville, LMSW

Cynthia (Cindy) is passionate about supporting pregnant and parenting women through the unique challenges of pregnancy. With decades of experience medical settings, she understands the vital connection between physical health and emotional well-being. In her role, she helps normalize the emotional, physical, and psychological changes that can feel overwhelming, recognizing that this journey isn’t always joyful or pleasant.
Cindy earned her master’s degree in Social Work from Western Michigan University and holds specialized training in perinatal mood disorders and infant and pregnancy loss, through Postpartum Support International. She feels privileged to be part of Bronson’s Behavioral Health Collaborative Care and finds fulfillment in empowering women to navigate this transformative time with confidence and compassion.
