- Supporting stroke patients and their families during recovery
- Reaching out during the hospital stay and after you return home
What does a stroke nurse navigator do?
Our nurse navigator works only with stroke patients and can help with:
- Understanding your diagnosis and treatment
- Connecting you and your family to stroke resources and support groups
- Working with your doctor to set reasonable goals
- Managing your medications
- Helping you stay connected to the healthcare system throughout recovery by phone or MyChart
- Preparing you to care for yourself at home
- Following your progress for up to one year after your stroke
- Guiding you and your family through the Bronson system
- Hosting the Stroke Survivor Support Group class and event details page for patients and caregivers
