- Reaching out to you and your and families during the hospital stay and after you go home
What role does our stroke nurse navigator play?
Our nurse navigator is dedicated only to stroke patients, and will help you with:
- Understanding your diagnosis and procedure
- Directing you and your family to stroke resources and support groups
- Working with your doctor to set reasonable goals
- Managing your medications
- Keeping you connected to the healthcare system throughout recovery by phone or MyChart
- Preparing you to care for yourself at home
- Following you for up to a year after your stroke
- Guiding you and your family through the Bronson system
- Hosting a monthly Stroke Survivor Support Group for patients and caregivers
