  • Do you have difficulty falling asleep?
  • Do you wake and have trouble falling back asleep?
  • Do you often wake up gasping for air?
  • Do you wake with a dry mouth?
  • Do you have high blood pressure?
  • Do you wake with morning headaches?
  • Do you struggle to stay awake during the day?
  • Have you fallen asleep while driving?
  • Are you irritable and depressed?
  • Do you have trouble concentrating?
  • Do you kick and move a lot at night?
  • Do you have leg cramps at night?
  • Do you feel an aching/crawling sensation in your legs?
  • Do you sleep upright or in unusual positions?
  • Do you have a history of cardiovascular disease, diabetes, gastroesophogeal reflux disease (GERD), or mood disorder?
  • Do you have a family history of Obstructive Sleep Apnea?
  • Do you have a family history of early heart attack or stroke?
  • Do you have a child with Attention Deficit Disorder (ADD) or Attention Deficit Hyperactivity Disorder (ADHD) symptoms?
  • Does your child have large tonsils?
